New Patient Registration

To register with our practice, you will need to complete two registration forms, which are available at reception or you can download them and hand into the Practice. All newly registered patients will be asked to attend the Health Care Assistant for a health check.  This will last for about 20 minutes and allows the Health Care Assistant to take note of any past medical history while we are waiting to receive your medical record. Please ensure this appointment is kept.

You must bring 2 forms of identification with you to a new patient medical. 

The Practice does not discriminate on the grounds of race, gender, class, age, religion, sexual orientation or appearance, disabling or medical condition. It is necessary for the practice to record ethnicity for all our patients in agreement with our contract with Greater Glasgow and Clyde Health Board.
